Private/Group Health Insurance
Central Care PT accept most group health insurance plans. It is our policy to confirm the eligibility and benefits of all of our patients and inform them of the particulars of their insurance coverage for physical therapy at the time of their first visit. We also work with the patient’s doctor and insurance company to obtain authorization for therapy when it is required.
We are happy to check your eligibility and benefits. Please provide us with your insurance company name, policy number, social security number, Driver's License number, date of birth and insurance phone number. Email it to info@centralcarept.com.
We do prefer that you find out what your chosen insurance plan provides you. Simply call the number on your card to determine your Deductible and Co-insurance/Co-payments. You will be responsible to pay for those items during each visit. Patients whose annual deductibles that has not been met are asked to pay for their visits until their deductible is met.
You always have to choice to pay out of pocket at each visit and receive a 30% discount on all services that we provide at same day. You may request for reimbursement from your insurance from all your expenses after successfully finishing treatment.
*Due to the poor reimbursement policies and low rates of most insurance plans, we currently choose to be out of network. We provide specialized care and treat all of our patients like family. The current insurance rates do not reflect the professionalism and patient satisfaction that we provide. We believe that our fast and cost-efficient services may save you more money because we guarantee long term results.
Auto Insurance
Patients who are involved in a motor vehicle accident may choose to use their automobile insurance if there is “Medical Pay” included in their policy. These amounts generally range from $1,000.00 to $20,000.00 depending on the policy. Central Care PT can bill the patient’s insurance up to the amount of “Medical Pay” included in their policy. Central Care PT also accepts liens while your litigation is pending.
Worker’s Compensation
Injured workers have 100% coverage for accepted claims through their employer’s worker’s compensation insurance. Due to new workers’ compensation regulations legislated by the California State Assembly, prior authorization for physical therapy may be required before the patient can begin therapy. Central Care PT work with the patient’s doctor and insurance company to obtain authorizations in as timely a fashion as possible.
Self-Pay
For patients without insurance or who choose not to use their insurance, we have affordable payment options available. We offer you 30% discount on Physical Therapy services and 70% discount on Occupational and Speech Therapy services if you wish to pay out-of-pocket at the time of your visit. We can also offer you a payment plan option for 90 days.
We accept payments by cash, check, and money order only.
